Kent Community Health Chronic Pain Service have been delivering Chronic Pain Services in 2 local prisons for a number of years. This has focussed on holistic pain assessments and medication rationalisation in line with NICE guidelines, Faculty of Pain Medicine and RCGP prison prescribing protocols.
The Service has evolved to a two tier model of care, with complex patients reviewed in a MDT consisting of pain specialist, GP and pharmacist and high risk medications like opioids and gabapentinoids being deprescribed. This is supported by a nurse led pain clinic where patient education and self management strategies are the focus.
In 2022 the service was commissioned to extend their services in to 2 new settings and the outcomes of the first year in practice have been discussed in this nomination.
