To support general practice restoration and recovery and restart paused work due to COVID-19, comprehensive support in the form of a pragmatic guide and suite of searches was provided to general practice in Derbyshire to successfully target and prioritise capacity to review patients with long term conditions.
The searches enable practices to continuously monitor patients with long term conditions to identify those where a review is outstanding and ensure prioritisation of patients with perceived greatest clinical need.
This supports practice planning for long term condition management and primary care workload.
